How to obtain Certificate of Birth – Stillborn

A Certificate of Birth Resulting in Stillborn (CBRS) can only be obtained through the Georgia Department of Public Health’s Office of Vital Records.

A CBRS can be issued for any “unintended, intrauterine fetal death,” at 20 weeks or more or of a birth weight of at least 350 grams.

To request a CBRS, send a letter to the Office of Vital records that includes the following information: the mother’s first and last name, the mother’s date of birth, the baby’s name (if given), the facility in which the event took place, and the number of copies requested.

A CBRS will include the following information (when available):

• Baby’s name as provided by the parents’ in their initial request for the CBRS. If no name is provided, the Office of Vital Statistics will fill in Baby Girl or Baby Boy and the last name of the parents.

• Date, time and place (hospital, county) of delivery

• Gender

• Mother’s name and residence

• Father’s name

• The attendant at delivery

• Gestational age and weight at delivery

• Type of delivery (single, twin, triplet, etc.)

• The words: “This certificate is not proof of live birth.”

Each copy of the CBRS is $10 and can be paid by certified check or money order and made payable to Vital Records. Mail the request and payment to: Vital Records, 2600 Skyland Dr NE, Atlanta, GA 30319.

A death certificate can be obtained through the hospital where the delivery occurred. A form is completed by the doctor and sent to the Office of Vital Records by the hospital. Parents can then contact the Vital Records office to receive the certificate.
